Who Should Make The Call?
Anyone can call Hospice; you, your family, your friend, your physician. Our hospice referral/ admission nurse will make a visit to determine the eligibility and special needs of the person with a life-limiting illness and their loved ones.
Where is Hospice?
Hospice of the North Country cares for those with a life-limiting illness in the comfort of their own home or chosen setting (hospital, nursing home, assisted living facility, adult care home) and in the company of family and friends.
