Hospice of the North Country will contact the patient's doctor to discuss if the patient is medically appropriate for hospice care. Referrals for hospice care can come from you, your loved ones, your own doctors, or a hospital.

We visit you, wherever you are living—at home, in a nursing home or at CVPH or Alice Hyde Medical Center—and work in tandem with your current physician to provide an extra level of specialized comfort.

At Hospice of the North Country, we understand that each patient’s situation is unique, which is why we develop an individualized plan of care to provide the right services to meet your specific needs. Your care plan is continually reviewed and updated by our medical director, your physician, and members of your clinical care team. Some of the services provided by the clinical care team are:

  • Physician care and support in collaboration with your current Physician

  • Nursing care and support to maximize comfort and minimize pain

  • Medication management for pain and symptom control

  • Medical equipment and supplies related to the life limiting disease

  • LPN to assist with personal care needs

  • Volunteer non-medical aid and support to give comfort and assistance

  • Social work and counseling about what to expect

  • Pastoral or spiritual care

  • Other specific treatments and therapies as needed

  • Bereavement services

Family Support

We understand that this may be a difficult time for you and your family, and we are here to help you. Our specially trained care team offers you personalized attention and support, and helps you and your family manage physically, emotionally and spiritually. 

We offer a wide range of services to support and assist the family members of our patients. We don't just help you learn how to care for your loved one; we also offer emotional support, assistance with physical care, help with filling out paperwork, and counseling.

In addition, we coordinate family meetings to discuss any changes in the patient's condition or the need for change in the care routine.

  • Our nurses are available 24-hours a day.

  • We offer caregiver respite to enable you to get back to your regular routine or run errands.

  • Families can expect a better quality of life for their loved one, since many symptoms that might otherwise require a hospitalization can be successfully managed by the hospice team in the patient’s home.

For those who have lost a loved one, the experience of grief can be very stressful and challenging. It may have a profound impact on all aspects of life.

At Hospice of the North Country, we understand the impact grief can have on family, friends, colleagues, and community. We are here to help. Hospice of the North Country is a well-respected regional resource for grief support and bereavement services. We serve families in our area and educate the community as a whole. Through individual counseling, telephone support, workshops and support groups, we can help you understand and manage the grieving process. 

  • We offer tailored programs and services to meet the needs of adults, teens, and children as they work through their grieving experience.

  • We hold non-denominational services and ceremonies for families and friends of hospice patients, and other caregivers and loved ones.

  • Grief Support Groups may be available to members of the community as well. Your loved one need not have been served by Hospice of the North Country to participate.

  • Grief support services are available at a minimum of 13 months following a loss.

The Hospice Nurse is the core of the Hospice Team. The Registered Nurses distinguish themselves from their colleagues in other nursing specialty practices by their unwavering focus on end-of-life care, by providing expert pain and symptom management, combined with compassionate listening and counseling skills. The nurse promotes the highest quality of life for the patient and family. Regardless of the setting, the hospice nurse strives to achieve an understanding of specific end-of-life issues from the perspective of each patient and his/her family.

Social Work

A Social Worker in the hospice setting is the professional who helps those diagnosed with a life limiting illness and their family to understand and navigate the multiple dimensions of dying. The Social Worker focuses on the internal and psychological coping skills of the individual and his/her family to support self-worth and symptom relief, most specifically the psychosocial components related to pain.

Spiritual Care

The Spiritual Care Counselor provides an inter-faith approach to pastoral care for individuals and families of those who are facing a life limiting illness. The care is based on understanding of current theological, spiritual, religious beliefs and practices among various religious groups. They may be called upon to provide: encouragement and hope, help in finding meaning and purpose to support the patient and family when they might be feeling alone. They offer comfort and can assist with reconciliation during times of helplessness. They may offer the comfort of prayer, talk about the struggles of life, and provide solace through sacred writings.

FAQs

 

Is Hospice just for cancer patients?

No. We help any patient with a terminal illness and a prognosis of less than six months. Cancer, kidney disease, Parkinson’s, ALS, multiple sclerosis, liver disease, HIV/AIDS, Alzheimer’s and dementia are on the short list of ailments our patients have.

Is Hospice just for the elderly?

We serve patients of any age, from children to the elderly. 

Do I have to be in my final days to be eligible for Hospice?

A patient is eligible for hospice care if their physician estimates that have six months or less to live. Almost all Hospice families say they wish they had entered the Hospice program sooner, so that they would receive more services enhancing their quality of life. 

Can anyone refer someone to Hospice?

Yes. Absolutely anyone.

What say will I have in my treatment?

You will be informed about all aspects of your care and your wishes will guide the hospice team in developing your personal care plan.  Your decisions are their directions. 

Where is Hospice care available?

We’ll help you in your home, a nursing facility, a hospital or wherever you are. We even help arrange care and support if you want to travel.

Will Hospice help me die or quicken my death?

No. Hospice care is meant to address the most demanding difficulties during advanced stages of an illness. Hospice affirms life as long as life lasts, neither hastening nor postponing death. The emphasis of Hospice is on enhancing the quality of life until death comes naturally.

Will I be able to continue with my own doctor?

Yes.  A Hospice medical director is available to consult with your doctor, and Hospice nurses will work with your doctor on all aspects of care.

Will I be house-bound?

Not because of Hospice care. In fact, our goal is to help you meet your goals. So if you want to go to a family gathering, attend some social event or even just do a bit of shopping, we’ll help make it happen.

Is Hospice available to people of all faiths?

Yes. We serve people of all faiths, religious backgrounds and beliefs. Our chaplains are available to help you with your own spiritual needs, if you desire.

How will I pay for Hospice?

Most health insurances, including Medicare and Medicaid, have hospice coverage.  No one is denied Hospice services because of inability to pay.
