Hospice of the North Country wants to be your partner and advocate in transitioning eligible patients to hospice care. We make the referral process, transition of care, and reporting extremely easy and convenient.
Whether your patient is ready for hospice care or just contemplating end-of-life care options—we welcome the opportunity to talk with patients and families about their options for medical care and support. We are happy to help with MOLST completion and end-of-life conversations. We will schedule an informational meeting – with no obligation and no strings attached. For patients who are ready to transition to hospice care, provide us with the patient’s information and we’ll schedule an admission visit.

Qualifying criteria.
Below is our qualifying criteria for admission to Hospice of the North Country:
Anticipated life expectancy of 6 months or less
Documented decline in clinical status
Curative options no longer desired or beneficial
Diagnosis-specific qualifiers to be considered
